– The expansion brings the total number of supported organizations to more than 2,400, serving nearly 3 million patients through value-based care programs across the United States.
Fueling Nationwide Adoption of Value-Based Care Models
Aledade partners with primary care organizations of all sizes, including over 300 community health centers, many of which are located in underserved areas. Aledade’s growth is driving significant adoption of value-based care models, particularly in the Medicare Shared Savings Program (MSSP). Notably, 83% of Aledade’s new MSSP participants are joining the program for the first time, highlighting the company’s role in expanding access to this innovative care model.
Aledade now supports over 2,400 primary care organizations in 46 states and the District of Columbia. The company serves nearly 3 million patients through various value-based care programs. Aledade is responsible for nearly one in five new practices participating in the MSSP.
