What does your organization do to hold poor leaders accountable? [closed]

New to this platform and am interested in writing a book on Organization and Leadership. I know, I know how many books have been written on this topic? The difference? This book will be written from the employee point of view. I will not quote anyone here either. In my experience, an employee who reports poor leadership is moved to another team, starts to decline in productivity (and is fired) or eventually ends up leaving (all 3 has happened to me). Short of sexual harassment or racism Managers do not seem to be held accountable for their actions. Thoughts?
